DSGM I. Special Manual Technique (aka SMT)

Its application aims at three different goals:

1. Relief from contracture

loosening retraction and adhesion of muscles and sinews;
re-establishing the natural condition of muscular, sinewy or connective tissues;
restoring their actual activity and effectiveness.

Regardless of its original cause, deficiency of the movement is accompanied by inactivity, which, in turn, is accompanied by contracture of the muscles and sinews. Muscles, including their sinewy origin or insertion, are enclosed by a thin tissue, the fascia. The fascia is mainly responsible for the contracture of the whole envelope of connective tissues, since it shrinks at any sign of deficiency or inactivity and obliges the muscular fibres into an area that is much smaller than it would be necessary for them to function appropriately. Later on, this unnatural condition is lodged and movement becomes a mechanical obstacle. This mechanical obstacle can be surmounted only by mechanical means, a manual performance aimed at loosening the fascia. Using the Dévény SMT, the fascia and other contracted parts between the muscular groups can be eased up. As a result, the fascia and the fibrous tissues surrounded by it can regain their size, their shape and thus their potencial activity.

Medication or passive dislodgement of the sinews by physical work-out, swimming, or hydrotherapy, prove far less effective than SMT, inasmuchas they all serve different aims.
- Medication is a chemical treatment which cannot work effectively against mechanical deficiency.
- Passive dislodgement is a replacement of the joints by means of an external force, which cannot decontract the muscles and the fascia.
- Active work-out proves indispensable for invigorating muscular activity and developing co-ordination, but has no effect whatsoever on the contracted fascia.
- Hydrotherapy uses buoyancy to facilitate movement, but does not restore the natural condition of muscular or sinewy tissues and it does not re-create the missing set of movements.

SMT is unlike the traditional technique applied for soft parts and, as far as its goal and procedure are concerned, it cannot be compared to massage. (For further details confer with the complete comparative study in Anna Dévény’s manual.)

2. Normalization of muscular position

Beside contracture, the physically handicapped tent to develop pathological postures and positions. Although the muscles may function properly, their pathological position entrains pathological movement, since the actual position determines the direction of the movement and causes the muscles to „go astray”. While loosening contracture, SMT aims at terminating pathological muscular position. 

3. Direct stimulation of the nervous system

DSGM considers the set of sinews as an autonomous system within the apparatus of muscles and sinews. Sinews constitute not only an origin or an insertion for the muscles, but also a peripheral part for the nervous system. With its end organs located in them, they are instrumental in transmitting stimuli through the spinal cord to the brain. Direct stimulation activates the brain, whose feed-back activates the muscles. A positive effect SMT can have on end organs is direct stimulation of the nervous system. Direct stimulation proves much more efficient than indirect stimulation. No indirect stimulation, such as physical exercises, swimming, intensive playing, etc., can have the same output as the direct stimulation of the nervous system. Physical exercises, swimming, intensive playing, etc. are crucial in developing movement with capable children, but prove insufficient for activating a missing set of movements or curing contracture. Also direct stimulation of the nervous system provides a highly efficient treatment for brain paralysis (CP). 

The result you should expect from DSGM treatment is normal movement and viability and not active engagement of the patient to no avail.
